Overview:
At Agape Care Group, we're dedicated to providing comfort and support through compassionate care and meaningful experiences. As a Licensed Practical Nurse (LPN), you'll play a vital role in delivering exceptional patient care in a hospice setting.

Responsibilities:
Conduct patient visits and provide routine care under the direction and supervision of the registered nurse.
Communicate effectively with the care team, patient, and their family to ensure seamless care coordination.
Work collaboratively with the interdisciplinary team to develop and implement patient care plans.

Qualifications:
Graduate of an accredited school of nursing with a current state license as a licensed practical nurse.
2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred).
Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
