Key Responsibilities- Conduct comprehensive assessments of patients and develop personalized care plans to address their unique needs and goals.
- Provide direct patient care, including medication management, wound care, and other interventions as needed.
- Collaborate with patients, caregivers, and other healthcare professionals to ensure seamless care coordination and communication.
- Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with the latest clinical research, guidelines, and best practices in home-based medical care.
- Current, unrestricted RN licensure in the state of practice.
- Current, unrestricted NP licensure in the state of practice.
- Completion of a Nurse Practitioner accredited program.
- Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
