Hospice Nurse

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team in providing exceptional care to our clients in Montgomery County. As a Hospice Nurse, you will be an integral part of a multi-disciplinary healthcare team, working closely with clients and their families to provide palliative care and support.
Responsibilities:- Make home visits to clients in designated territories, providing medical care and support as needed.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures, as well as comfort care.
- Monitor clients' conditions, reporting changes to Clinical or Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- A current license as a Registered Nurse (RN) in Pennsylvania.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience is strongly preferred, but not required.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
