About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team at FC Compassus LLC. As a Home Health Occupational Therapist, you will work one-on-one with patients in their homes, empowering them to live safely and productively.

Key Responsibilities
  • Assess patients' needs and develop personalized treatment plans to improve their daily activities and quality of life.
  • Work collaboratively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Provide education and training to patients and caregivers on adaptive techniques, equipment use, and home modifications.
  • Conduct thorough evaluations and assessments to identify patients' strengths and areas for improvement.
  • Develop and implement effective treatment plans that address patients' physical, cognitive, and emotional needs.
Requirements
  • Bachelor of Science in Occupational Therapy.
  • Current State License as an Occupational Therapist.
  • Valid Driver's License.
  • Current CPR, negative TB screen, and Hepatitis consent/declination.
  • Two years' experience as an Occupational Therapist in an acute care or rehabilitation setting.
Preferred Qualifications
  • Experience working with patients with diverse needs and conditions.
  • Knowledge of durable medical equipment and assistive technology.
  • Strong communication and interpersonal skills.
