LPN Home Care Specialist

As a Licensed Practical Nurse (LPN) at Constellation Health Services, you will play a vital role in providing high-quality care to our patients in their homes or community settings. We are seeking a dedicated and compassionate LPN to join our team and contribute to our mission of delivering exceptional patient care.
Key Responsibilities:
Provide nursing care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
Implement current nursing practice following a comprehensive assessment and the plan of care.
Maintain accurate and timely documentation of patient services to reflect the plan of care.
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse.
Requirements:
Graduate of an accredited practical nurse program.
Licensed New York State Licensed LPN with a minimum of 1 year of clinical experience.
Community health home care/hospice or medical/surgical experience is preferred.
Clinical auditing background preferred.
Experience conducting chart reviews preferred.
Wound care experience preferred.
