Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Frankfort Terrace, an Intermediate Care Facility (ICF) providing care to the mentally ill/behavioral health population.

Key Responsibilities
  • Provide excellent clinical care to patients, working collaboratively with a multidisciplinary team to help them reach their clinical goals.
  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each patient.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to promote optimal patient outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Current RN or LPN license in IL.
  • Prior experience in a SNF or ICF preferred.
  • Prior experience or desire to work with behavioral population.
  • New Graduate Nurses welcome to apply.
  • Electronic medical records experience is helpful (Point Click Care or PCC is ideal).
