About the Role

As a Home Health Registered Nurse with Interim HealthCare, you will have the opportunity to provide high-quality patient care in a dynamic and flexible environment. Our team is dedicated to delivering exceptional patient outcomes, and we are seeking a skilled and compassionate RN to join our team.

Key Responsibilities
  • Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with a multidisciplinary team, including physicians, LPNs, CNAs, and aides, to ensure seamless patient care.
  • Conduct thorough patient assessments, document progress, and report changes to the patient's physician.
  • Develop and implement personalized care plans, educating patients on medication management, therapy, diet, and exercise.
  • Coach family members on patient care, medication management, and home safety.
  • Ensure patient goals are met and coordinate discharge from services.
Requirements
  • Graduate of an accredited nursing program and active RN license in IL.
  • Minimum of 2 years of nursing experience, preferably in home healthcare.
  • CPR certification and BLS certification.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion.
