Clinical Liaison Representative

As a Clinical Liaison Representative at Spaulding Hospital-Boston & Cambridge (SRN, SRH, SHC), you will play a critical role in ensuring a smooth transition of patients to assigned facilities. Your primary responsibility will be to perform preadmission patient evaluations, coordinate the referral process, and work with facilities to remove barriers to create a seamless transition of care.
Key Responsibilities
- Coordinate and manage the referral process to ensure timely transitions of patients to SRN or PHH facilities.
- Obtain patient assessment information from various sources and transmit data to appropriate systems.
- Develop and maintain collaborative relationships with internal and external staff to ensure customer satisfaction.
- Nurture referral source relationships to ensure customer satisfaction.
- Support challenging transfers using SRN/PHH clinical and supervisory resources.
- Qualify patients for admission and ensure adherence to SRN/PHH criteria.
- Consider discharge plan alternatives to determine the best plan for patients' needs.
Requirements
- Current license in Massachusetts as RN, PT, OT, SLP (IRF requirement), LTAC, SNF may include LCSW; LICSW, MSW, BSW, LPN, PTA, COTA, RT.
- Knowledge of IRF LTAC, SNF, and/or Home Care preferred.
- Excellent computer skills with working knowledge of Microsoft Office Suite and ability to learn other software applications.
- Broad knowledge of various insurances and government reimbursements.
- Strong written, verbal, and electronic communication skills.
- Highly developed interpersonal and relationship-building skills, working with a diverse population.
- Ability to organize and prioritize effectively while working independently and adapting to changing conditions.
- Effective problem-solving skills to meet demands of all parties.
- Comfortable working in a fast-paced environment.
- Ability to make appropriate clinical decisions based on information provided independently.
