Job Summary

Amedisys is a leading provider of home health, hospice and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system.

Responsibilities:
  • Conduct comprehensive patient assessments and collaborate with the care team to develop and implement a personalized plan of care.
  • Make referrals to other disciplines as indicated by the patient's identified needs or document the rationale for not doing so.
  • Promote patient health and independence through education and rehabilitative measures, assisting patients in learning self-care techniques.
  • Supervise licensed practical nurses (LPNs) and home health aides (HHAs).
  • Ensure timely and accurate documentation of patient care.
  • Regularly communicate patient progress to the clinical manager and care team.
  • Develop and implement staff education programs.
  • Participate in on-call responsibilities and provide on-call services to patients and families as assigned.
  • Engage in ongoing clinical development and continuing education programs.
Requirements:
  • One year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
