A Certified Nursing Assistant plays a crucial role within the healthcare team, delivering exceptional care to residents while adhering to established standards, philosophies, and resident rights.
REPORTING STRUCTURE:
This position reports directly to the Nurse on duty or Supervisors, with annual performance evaluations conducted by the Director of Nursing or Assistant Director of Nursing, incorporating feedback from Unit Nurses.
KEY RESPONSIBILITIES:
1. Actively engages as a collaborative team member, demonstrating respect, dignity, accountability, professionalism, and ethical values towards residents, families, and colleagues while ensuring confidentiality.
2. Adheres to the established hierarchy and performs all assigned duties within the scope of practice.
3. Attends to residents' Activities of Daily Living (ADLs), including personal hygiene, oral care, bathing, grooming, and dressing, in accordance with policies.
4. Understands and implements the individualized care plans focusing on the physical and emotional well-being of each resident, addressing mobility, hygiene, and emotional support as directed.
5. Possesses knowledge of Resident Rights and the policies regarding abuse and neglect, including reporting protocols.
6. Complies with safety policies and demonstrates competency in equipment usage, including managing supply needs.
7. Consistently adheres to Infection Control protocols, including hand hygiene, maintaining a clean environment, and following standards for all resident care areas.
8. Supports residents' elimination needs per policy, documenting toileting times and reporting on incontinence and continence.
9. Implements Rehabilitation Nursing strategies, encouraging independence, assisting with mobility, and ensuring proper body mechanics during transfers.
10. Reviews the activity calendar at the start of each shift to ensure timely transportation of residents to activities.
11. Assists with admissions, discharges, transfers, and transportation to appointments.
12. Reports any changes in residents' conditions, including signs of illness, injuries, or accidents, to the Nurse on duty.
13. Measures, records, and documents vital signs, height/weight, intake/output, and other relevant information for the resident care plan, reporting any abnormal findings to the Nurse.
14. Participates in resident care conferences and contributes positively to the care planning process.
15. Adheres to all personnel policies regarding attendance, breaks, meals, staff meetings, and dress code.
16. Schedules may vary based on departmental needs to ensure high-quality resident care.
17. Maintains a thorough understanding of Corporate Compliance regulations and promptly reports any potential violations.
18. Demonstrates knowledge of Continuous Quality Improvement principles and participates in relevant processes to meet customer service needs.
19. Upholds the highest ethical standards concerning discretion and confidentiality.
20. Provides age-appropriate care to adult and geriatric residents.
21. Attends all mandatory training programs, including successful completion of orientation and maintaining 12 Continuing Education Units (CEUs) annually.

EDUCATION AND TRAINING:
Must be certified as a Nurse Aide with the New York State Department of Health. Current Heartsaver certification is required.
EXPERIENCE:
Previous experience as a Nursing Assistant is preferred.
QUALIFICATIONS:
Exhibits professionalism, teamwork, compassion, and concern for residents and colleagues.
SCHEDULE:
Availability to work weekends, holidays, and various shifts as necessary.
PHYSICAL AND MENTAL DEMANDS:
This role requires standing and walking for extended periods, as well as lifting, moving, positioning, and transporting residents and equipment.
WORKING ENVIRONMENT:
Regular exposure to unpleasant odors, sights, and bodily fluids.
HAZARDS:
Potential exposure to contagious diseases and risks of injury from residents.
