Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.

Responsibilities
  • Administer medications and treatments to residents in accordance with their individualized care plans and physician orders.
  • Conduct regular medication reviews to ensure accuracy and completeness of medication administration records.
  • Collaborate with the Resident Care Director and Wellness Nurse to ensure timely and accurate medication administration.
  • Participate in the development and implementation of individualized care plans for residents.
  • Conduct regular audits of medication carts and storage areas to ensure compliance with regulatory requirements.
  • Provide education and training to staff members on medication administration and management.
  • Participate in quality improvement initiatives to enhance the quality of care provided to residents.
Requirements
  • Current LPN licensure in the state of Ohio.
  • Minimum of 1 year of experience in a long-term care setting.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Ability to work collaboratively as part of a team to achieve common goals.
