Key Responsibilities- Supervise and Coordinate Clinical Services: Oversee the delivery of clinical services to patients in their homes, ensuring that care is provided in accordance with physician orders and applicable laws and regulations.
- Lead Interdisciplinary Team: Coordinate and supervise a team of clinicians to ensure the continuity of high-quality care to home health patients.
- Assess Patient Needs: Continuously assess patient needs and ensure that care rendered is individualized to patient needs, appropriate, and reasonable.
- Develop and Implement Patient Plans of Care: Oversee the development, implementation, and updates to individualized patient plans of care, as appropriate.
- Communicate with Clinicians and Physicians: Communicate with clinicians and physicians to ensure that patient updates and new orders are implemented in a timely manner.
- Coordinate Care with Other Healthcare Providers: Coordinate all aspects of care with other healthcare providers, including durable medical equipment providers, caregivers, and transferring facilities.
- Review and Evaluate Care Delivery: Review and evaluate the delivery of care to ensure that it is high-quality, efficient, and effective.
- Participate in Quality Assurance and Performance Improvement: Participate in quality assurance and performance improvement activities to ensure that care is delivered in accordance with best practices and regulatory requirements.
- Current RN Licensure: Current RN licensure in the state of practice.
- CPR Certification: Current CPR certification.
- Driver's License and Vehicle Insurance: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
