Job Description:
We are seeking a skilled Medical Social Worker to join our interdisciplinary care team. The ideal candidate will have a Master's degree in Social Work, experience in healthcare settings, and excellent communication and interpersonal skills. Responsibilities include assessing patient needs, developing personalized care plans, providing emotional support, connecting patients with community resources, and collaborating with healthcare professionals to optimize patient outcomes.

Required Skills and Qualifications:
- Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social worker licensure in the state of practice (if required by state law or regulation)
- Valid driver's license, auto insurance, and reliable transportation
- Proof of current CPR certification
- Minimum one year of experience as a social worker in a healthcare setting, home health, and/or hospice
- Excellent oral and written communication and interpersonal skills
- Ability to organize tasks, develop action plans, set priorities, and function under stressful situations
