A highly skilled Case Manager RN is sought by Parrish Healthcare to join our dedicated team. As a Care Coordination Specialist, you will play a vital role in ensuring seamless patient care and efficient hospital operations.
About the Role:This is a full-time position with standard hours of 40 per week, from 8am to 4:30pm. The successful candidate will be responsible for managing and coordinating total patient care plans from admission to discharge.
Key Responsibilities:- Assess and evaluate patient plans of care within 24 hours of admission and at least every 96 hours, utilizing Milliman criteria to determine appropriate patient status and medical necessity of hospital admission.
- Document and convey relevant information regarding patient admission and continued hospitalization to various payors for authorization and reimbursement purposes.
- Collaborate with physician advisors when unable to resolve issues with attending physicians.
- Meet with patients and/or family representatives to assess, evaluate, and identify discharge needs, providing support and information as needed.
- Collaborate with healthcare teams to develop, plan, and facilitate safe and realistic discharge plans, adjusting as necessary throughout hospitalization.
The ideal candidate will possess a Bachelor's or Associate's Degree in Nursing, with a State of Florida RN License. A minimum of 2-3 years of experience in case management or a related field is also required.
