BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full-time opportunity performing home health visits. This position involves providing rehabilitative care and skilled nursing to adult and geriatric clients.

Key Responsibilities:

  • Conduct home visits within designated areas to provide personalized care and support.
  • Perform diagnostic assessments and measurements to inform care plans.
  • Develop and implement individualized care plans to meet the unique needs of clients.
  • Continually assess and revise care plans to ensure optimal outcomes.
  • Educate clients and family members on health and wellness strategies to promote independence.
  • Accurately document observations and evaluations to inform care decisions.

Requirements:

  • Current MD Physical Therapist - PT license.
  • Minimum of one year of recent PT experience.
  • Graduation from an approved program.
  • Strong interpersonal and organizational skills to work effectively with clients and colleagues.
  • Solid computer skills to navigate electronic health records and other systems.
