We are seeking a skilled Physical Therapist to join our team at Genesis Home Care. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
About the RoleAs a Physical Therapist, you will be responsible for providing skilled physical therapy services to patients as ordered by the attending physician. You will work closely with our interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team
- Conduct OASIS assessments to identify patient needs and develop plans of care
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
- Supervise and educate physical therapy assistants and nurse aides as needed
To be successful in this role, you will need to have a current license to practice as a Physical Therapist in the state of employment. You will also need to have a valid driver's license and auto insurance. We are looking for a compassionate and patient individual who is able to work independently and as part of a team.
