About the Role

We are seeking a highly skilled Social Worker to support our housing with supportive services program in Philadelphia, PA.

Job Summary:

  • Conduct comprehensive assessments and develop individualized care plans for clients dealing with substance abuse, homelessness, and behavioral health challenges.
  • Provide counseling and support services to promote recovery and stability.
  • Collaborate with a multidisciplinary team to deliver comprehensive care.

Key Responsibilities:

  • Facilitate access to community resources, including housing, medical care, employment services, educational opportunities, and financial management.
  • Maintain detailed records of client progress and services provided.
  • Advocate for clients' rights and needs within the community and with service providers.

Requirements:

  • Bachelor's degree in social work from an accredited institution (preferred).
  • Minimum 3 years of experience working with adults in substance abuse, homelessness, and behavioral health settings.
  • Excellent assessment, counseling, and case management skills.
