The Nurse HBPC Case Manager is a professional who provides care to a panel of patients enrolled in the HBPC program, coordinating care through case management to deliver high-quality and cost-effective healthcare.
Responsibilities- Provides direct patient care, assuming responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Administers vaccinations and procedures per established policies and guidelines.
- Influences care outcomes by collaborating with members of the interdisciplinary team.
- Participates in unit/program level quality improvement process and initiatives, as well as customer service programs.
- Provides care through home visits, phone calls, and general coordination of care using resources available within the program and this VAMC.
- Collaborates effectively with other program staff and healthcare professionals to deliver care.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
- Two to five years of professional nursing experience, including experience providing nursing care for patients of the geriatric population, including case management for chronic disease states.
- Master of Science Degree in Nursing preferred.
- Experience with technology, including electronic medical records and Microsoft Office programs, including Outlook, Word, Excel, and Teams.
- Competitive salary, regular salary increases, potential for performance awards.
- 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
- Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.
