About this role:

This position involves serving as an In-Home Care Provider for Clover Health. As a key member of our clinical team, you will be responsible for delivering high-quality patient care in a home-based setting. You will work closely with other healthcare professionals to develop and implement individualized care plans that cater to the unique needs and goals of each patient.

Responsibilities:

  • Provide comprehensive medical care to patients in their homes, utilizing advanced technology and evidence-based protocols to drive excellent outcomes.
  • Develop and maintain strong relationships with patients, their families, and caregivers to ensure seamless coordination of care.
  • Collaborate with multidisciplinary teams, including physicians, nurse practitioners, social workers, and other healthcare professionals to deliver holistic care.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to ensure delivery of exceptional care.
  • Participate in ongoing education and training to enhance your skills and knowledge in areas such as geriatric medicine, palliative care, and population health management.
  • Contribute to quality improvement initiatives, providing insights and recommendations to enhance patient care and service delivery.

Requirements:

  • Board Certification or Board Eligibility in Family Medicine, Internal Medicine, Geriatric Medicine, or Palliative and Hospice Medicine.
  • A valid New Jersey medical license and DEA registration.
  • Prior experience in house calls or home-based care settings is highly desirable.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • A strong commitment to delivering compassionate, patient-centered care that prioritizes the unique needs and goals of each individual.
