About the Role

We are seeking a skilled Nurse Practitioner to join our team at Enhabit Home Health & Hospice. As a key member of our interdisciplinary team, you will provide high-quality medical care to our patients and play a vital role in determining their eligibility for hospice services.

Responsibilities
  • Provide medical care to hospice patients, including history and physical exams, diagnosis and treatment of medical problems, and education and counseling of individuals, families, and groups.
  • Participate in the care of the hospice patient as a member of the interdisciplinary team.
  • Under the supervision of the hospice medical director and in collaboration with one or more physicians, perform face-to-face patient visits to assist in determining hospice eligibility.
Qualifications
  • Must be a licensed Nurse Practitioner (NP) in the state in which they currently practice.
  • Must have at least five years of nursing experience, with a minimum of one year in hospice preferred.
  • Must possess a valid state driver's license and maintain automobile liability insurance as required by law.
  • Must be able to safely drive an automobile in all types of weather conditions.
  • Must possess CPR certification for the healthcare provider.
