We are seeking a skilled pediatric nurse to provide high-quality clinical home care to medically fragile children in San Antonio, TX. As a pediatric nurse with Thrive Skilled Pediatric Care, you will have the opportunity to work with a leading provider of skilled pediatric home care and make a meaningful difference in the lives of our patients.

**Job Summary**

* Provide high-quality clinical home care to medically fragile children
* Work collaboratively with our interdisciplinary team to develop and implement individualized care plans
* Utilize your skills and expertise to provide exceptional patient care and support

**Responsibilities**

* Assess patient needs and develop individualized care plans
* Provide skilled nursing care, including tracheostomy and gastrostomy tube care
* Collaborate with other healthcare professionals to ensure comprehensive patient care
* Educate patients and families on disease management and self-care techniques

**Requirements**

* Current valid license to practice as a Licensed Vocational Nurse (LVN) in the State of Texas
* Experience working with medically fragile children
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
