Job Summary

Community Care Physicians is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a family practice setting. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our medical office.

Key Responsibilities
  • Administer and record vaccines, ensuring accurate and timely documentation.
  • Document verbal conversations with patients, maintaining confidentiality and discretion.
  • Accurately and efficiently document patient information into the electronic medical record.
  • Maintain a smooth patient flow, ensuring timely and efficient care.
  • Prepare patients for visits, adhering to office and provider requirements.
  • Collect patient specimens, such as urine and blood samples, as directed by the provider.
  • Enter orders accurately and timely into the laboratory system.
  • Perform clinical office procedures, including PFTs, EKGs, nebulizers, medication administration, and immunizations, as directed by the provider.
  • Process patient prescriptions, ensuring accuracy and efficiency.
  • Discuss test results with patients, following provider instructions.
Requirements
  • Valid NYS LPN license.
  • Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
  • Strong computer skills, including experience with electronic medical records (EMRs).
  • Knowledge of basic nursing principles and protocols.
  • Understanding of policies and procedures related to infection control, environmental safety, and patient confidentiality.
  • Maintain strict confidentiality and discretion.
  • Knowledge of medical chart reporting and records.
  • Skill in appropriate assessment and assistance techniques.
