Hospice LPN Staffing Pool Opportunity
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As a LPN, you will provide direct patient care, coordinate with the interdisciplinary team, and ensure that patients and their families have an optimal care experience.
Responsibilities:
- Provide direct patient care, including pain and symptom management
- Implement nursing practice following a comprehensive assessment and plan of care
- Document patient services accurately and timely
- Assess and educate patients and their families/caregivers
- Participate in coordination of hospice services
- Ensure compliance with state and federal legal and regulatory requirements
Qualifications:
- Graduate of an accredited practical or vocational nursing program with current licensure
- 1 year nursing experience required, 3+ years preferred
- Experience in community health, hospice, or med/surg preferred
