Job Summary:

We are seeking a skilled Medical Case Manager to join our team at Leidos. As a Medical Case Manager, you will be responsible for providing case management services to injured, ill, or recovering clients. This includes establishing specific rehabilitation objectives and goals, defining associated actions, and monitoring and evaluating rehabilitative progress.

Key Responsibilities:

  • Provide clinical case direction and establish specific rehabilitation objectives and goals.
  • Monitor and evaluate rehabilitative progress and obtainment of goals.
  • Accurately collect and record utilization metrics and provide monthly reporting.
  • Assess requirements for durable medical equipment, physical therapy, occupational, and rehabilitative services for clients.
  • Utilize electronic medical records and telemedicine technologies.
  • Be a skilled communicator and assist clients in understanding administrative requirements for reimbursements.
  • Perform ad hoc duties as per the Program Manager's discretion.
  • Pursue job accommodations as appropriate and understand the process for gaining approvals.
  • Formulate timely case reports, including presentation of factual information and analysis.

Requirements:

  • Ability to obtain a TS/SCI with polygraph clearance, therefore, US Citizenship is required.
  • Bachelor's degree in Nursing or a Medically Related Field and 5 years of previous case management experience, preferably in military treatment facilities.
  • Must be a Registered Nurse, Board-Certified Nurse Practitioner, or Board-Certified Physician Assistant.
  • Experience with care delivery, psychosocial support, and reimbursement systems.
  • An understanding of rehabilitative strategies.
  • Ability to problem solve complex medical issues.
  • Ability to establish working relationships and effective communication with professional personnel of various backgrounds.
  • Ability to communicate clearly, both verbally and in writing.
  • Previous experience in writing factual, clear, and concise reports, including situational analysis.
  • Ability to maintain daily scheduled activities and effective time management skills.
  • Decisive and efficient decision-making skills.

Preferred Qualifications:

  • Desired to have and maintain a nationally recognized current board-certification as a Certified Case Manager (CCM) from the Commission for Case Management Certification.
