Job Summary

We are seeking a skilled Occupational Therapist to join our team at Revival Home Health & Hospice. As a Home Health Occupational Therapist, you will work with patients in their homes to provide safe and effective care, promoting independence and quality of life.

Responsibilities
  • Assess and analyze patient care needs to establish a comprehensive plan of care
  • Develop and implement individualized therapy plans to meet patient goals
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Provide education to patients and caregivers on therapy techniques and safety measures
  • Document patient progress and outcomes in accordance with agency guidelines
Requirements
  • Graduate of an Occupational Therapist Education program approved by ACOTE
  • Current Occupational Therapist licensure in State and American Heart Association CPR certification
  • Two (2) years of experience in adult occupational therapy, home health experience preferred
