Job Summary

We are looking for a dedicated and empathetic individual to provide ongoing support and guidance to our clients. As a Mentorship Specialist, you will be responsible for developing and implementing personalized reintegration plans, providing daily phone support, conducting home visits, and assisting clients with medication adherence and life skills coaching.

Key Responsibilities

  • Develop and implement personalized reintegration plans for clients
  • Provide daily phone support and conduct home visits as needed
  • Assist clients with medication adherence and life skills coaching
  • Collaborate with clients' clinical providers to ensure comprehensive care
  • Document all sessions and interactions with clients
  • Participate in regular team meetings to discuss client progress and develop strategies for improvement

Requirements

  • Bachelor's degree in a related field (e.g. social work, psychology, counseling)
  • 1 year of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and clean driving record
  • CPR certification
