Job Overview

Curana Health is a leading provider of value-based primary care services for the senior living industry. We are seeking a highly skilled and compassionate Care Coordinator to join our team.

Key Responsibilities

  • Coordinate and attend facility and Curana Health educational/social events to foster strong relationships with key stakeholders.
  • Act as a liaison between nursing, sales, wellness, and leadership teams to ensure seamless communication and care coordination.
  • Assist with front desk duties, including appointment management, scheduling, and other related tasks.
  • Monthly outreach with residents and family members to assess needs and coordinate care consistent with provider's plan of care.
  • Assist with clinic setup and design within Curana Health specifications.
  • Ensure proper collateral and supplies are ordered in a timely manner.
  • Provide exceptional patient care, including greeting patients, family members, and staff with a smile, taking vital signs, and assisting with patient preparation.
  • Serve as backup to Medical Assistant in Curana Health Clinic.

Requirements

  • High School Diploma or equivalent.
  • Experience in the senior living setting is strongly preferred.
  • Outgoing individual with excellent interpersonal skills.
  • Organized and effective communicator.
  • Concierge or hospitality experience is a plus.
  • Proficient in Microsoft Office.
  • Positive attitude and strong interpersonal skills.
  • Passionate about helping others.
  • Highly organized.
  • Willingness to learn and be a part of something great.
