Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LPN to join our team of healthcare professionals. As a Private Duty Nurse LPN, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from hospital to home and providing continuous care to ensure their well-being.
Responsibilities- Implement physician-prescribed plans of care for patients
- Conduct patient assessments and coordinate care with healthcare teams
- Administer prescribed medications, treatments, and therapies as needed
- Document care provided in a timely and compliant manner
- Teach and train family members on health promotion and disease management
- Accept and write verbal orders from physicians as required, with co-signature from Case Manager
- Follow up with physicians, clients, family members, and healthcare teams on patient concerns and changes in condition
- Active, unencumbered LPN license in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- At least six months of prior hands-on nursing experience (preferred but not required)
- Reliable transportation
