Position Overview

Home Care Assistant

At WelbeHealth, we take a unique approach to caregiving. Our Home Care Assistants play a crucial role in enhancing the lives of our participants by delivering compassionate support in their residences. As integral members of our interdisciplinary team (IDT), they contribute significantly to the care planning process.

Under the supervision of the Home Care Supervisor, the Home Care Assistant is responsible for ensuring the safe provision of personal care services to participants in their homes, as per the established schedule and directives.

Key Responsibilities:

  • Deliver hands-on personal care and assist with activities of daily living (ADLs) for participants, which may include bathing, grooming, dressing, toileting, and safe physical transfers.
  • Support participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Record and monitor participants' vital signs as directed.
  • Assist with light meal preparation and feeding, ensuring participants receive adequate nutrition and hydration.
  • Facilitate safe transfers in and out of the home and provide medical escort services to appointments as needed.
  • Support the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.
  • Provide translation services when qualified.

Qualifications:

  • High School Diploma or equivalent.
  • Current Basic Life Support (BLS) / First Aid Certification is mandatory.
  • Preferred certifications include Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
  • Bilingual candidates are preferred.
  • Must be registered with the Department of Social Services Home Care Registry prior to employment.
  • Reliable transportation is required.

The salary range for this position is $22.93 per hour, plus bonuses and equity. WelbeHealth provides a competitive total rewards package, including 401k matching, healthcare coverage, and a variety of other benefits. Actual compensation will be adjusted based on experience and qualifications.
