Job Summary

We are seeking a Licensed Clinical Social Worker to join our team at Valor Healthcare. As a key member of our mental health services team, you will provide a range of services including psychosocial assessments, treatment planning, and case management.

Responsibilities
  • Conduct psychosocial assessments and develop treatment plans for adult patients
  • Provide short-term, solution-focused counseling and education on resources and benefits
  • Collaborate with the core PACT Team and expanded PACT Team to develop patient care plans
  • Develop program content and lead activities to enhance social development and group goals
  • Provide case management and referrals as needed
  • Conduct group therapy sessions, including PTSD groups, weight management, and anger management
  • Help members develop attitudes and social skills for improved family relations and community responsibility
  • Maintain confidentiality of veterans' data and information
  • Comply with all VA and company training requirements
Requirements
  • Master's degree in Social Work from a program registered by the Department of Education
  • Valid, unrestricted LCSW/MSW license in the state of desired employment or any U.S. state or territory
  • Minimum of 2+ years' post-master direct counseling experience with adult patients
  • Proven mastery of advanced sociological, psychological, and mental health care related interventions and issues
  • Certification in Basic Life Support and ACLS
  • Proficiency in written and spoken English
  • Strong computer skills, including EMR experience
