Job Title: Travel Nurse RN Home Health

Epic Travel Staffing is seeking a skilled Travel Nurse RN to join our team in providing high-quality care to patients in their homes.

Job Summary:

We are looking for a dedicated and compassionate Travel Nurse RN to work in our Home Health department. As a Travel Nurse RN, you will be responsible for providing patient-centered care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:
  • Provide high-quality patient care in the home setting, including assessments, interventions, and education.
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
  • Collaborate with the interdisciplinary team to coordinate care and services.
  • Document patient progress and outcomes in accordance with regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current RN licensure in Maine.
  • At least 1 year of experience in Home Health or Hospice care.
  • Experience with EPIC EMR system.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
