Community Health Worker

About the RoleThis is a field-based position with a home-based office, requiring local travel up to 50% and mileage reimbursement at the current government rate. You'll work Monday through Friday, 8 am to 5 pm, with alternative schedules available after 6 months of employment.
Key Responsibilities- Assess, plan, and implement care strategies tailored to individual patient needs, ensuring the most appropriate and least restrictive level of care.
- Utilize company and community-based resources to establish a safe and effective case management plan for members.
- Collaborate with patients, families, and healthcare providers to ensure quality coordinated care and services.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Communicate with stakeholders to ensure timely and effective delivery of health-related information.
- Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.
- Document all member assessments, care plans, and referrals provided.
- Bachelor's Degree in a health-related field or social work, or Active Licensed Practical Nurse (LPN) license.
- 1+ years of field-based experience.
- 1+ years of knowledge of community resources, culture, and values.
- Intermediate computer proficiency, including Microsoft Outlook and Teams.
- Valid Driver's license, current automobile insurance, and reliable transportation.
- Ability to travel locally, up to 100 miles round trip, and up to 50% of the time.
- Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information.
- Community Health Worker (CHW) Accreditation.
- Experience working in Managed Care.
- Knowledge of Medicaid/Medicare population.
