The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents in our Skilled Nursing Unit, Short Term Rehab Unit, or Dementia Care Unit. As a member of our team, you will be responsible for delivering exceptional care, working closely with residents, family members, and healthcare professionals to ensure the best possible outcomes.

Key Responsibilities:
  • Provide outstanding care to a group of residents/patients, identifying individual needs and developing personalized care plans.
  • Notify the charge nurse and/or physician of any changes in a resident's condition, ensuring timely interventions and adjustments to care.
  • Make daily rounds to assess residents' needs, updating care plans as necessary and required.
  • Administer medications and treatments, monitor intravenous fluids, and manage medication orders from the pharmacy.
  • Communicate effectively with residents, family members, and healthcare professionals, making referrals to other departments when appropriate.
  • Maintain accountability for controlled substances, inventory medications, and report discrepancies to supervisors.
Physical Requirements:
  • Frequently communicate with employees, residents, family members, and healthcare professionals.
  • Occasionally lift objects weighing up to 50 pounds.
  • Frequently remain in a standing position, stooping, kneeling, or bending to assist residents and coworkers.
  • Use all body limbs up to 100% capacity.
Requirements:
  • 2-3 years of experience as a licensed nurse in areas including Skilled Nursing Facility, Dementia Care, Short Term Rehabilitation, or Orthopedics.
  • Active KY Licensed Practical Nurse License.
