You will be part of a team of healthcare professionals who are committed to delivering exceptional care to our members. Our Medical Social Workers are responsible for implementing standards of care for medical social work services, ensuring that our patients receive the highest level of care possible.
Key Responsibilities:- Assess the psychosocial status of patients related to their illness and environment, and communicate findings to the registered nurse.
- Carry out social evaluations and plan interventions based on evaluation findings.
- Maintain clinical records on all patients referred to social work.
- Provide information and referral services for Physicians Choice Home Health patients and families/caregivers regarding practical and environmental needs.
- Provide information to patients or families/caregivers and community agencies.
- Serve as a liaison between patients or families/caregivers and community agencies.
- Maintain collaborative relationships with Physicians Choice Home Health personnel to support patient care.
- Maintain and develop contracts with public and private agencies as resources for patient and Physicians Choice Home Health personnel.
- Participate in the development of the total plan of care and case conferences as required.
- Assist physician and other team members in understanding significant social and emotional factors related to health problems.
- Participate in discharge planning.
- Participate in inservice education programs.
- Other duties as delegated by the Clinical Supervisor.
