About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Hospice of the Midwest, a part of BrightSpring Health Services. As a Hospice Care Specialist, you will play a vital role in providing personalized care to patients in their homes, supporting their physical, emotional, and spiritual needs.

Key Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming, as assigned by the patient's care plan.
  • Assist with daily living activities, such as feeding, toileting, and transferring patients.
  • Monitor and report any changes in the patient's condition to the Case Manager.
  • Assist with light housekeeping and laundry tasks to maintain a clean and comfortable living environment.
  • Participate in quality assessment and performance improvement activities to ensure high-quality care.
Requirements
  • Meet the training requirements of the State, including a minimum of 75 hours of classroom and supervised practical training.
  • Have a minimum of 6 months of experience in providing personal care, including experience working with the terminally ill.
  • Be at least 18 years old and able to read and follow written instructions.
  • Have a strong understanding of hospice philosophy and be comfortable providing specialized care to patients with terminal illnesses.
  • Be able to work with minimal direct supervision and possess strong communication and organizational skills.
