Job Description
As a registered nurse in this role, you'll be responsible for providing and directing the delivery of nursing care to patients in their homes. This will involve conducting initial and follow-up visits, assessing patient needs, developing individualized care plans, and ensuring that all necessary documentation is completed accurately and in a timely manner.

Responsibilities
• Assess patient needs and develop personalized care plans;
• Provide direct patient care as prescribed by physicians;
• Communicate effectively with patients, family members, and healthcare team members;

Requirements and Qualifications
• Current licensure as a registered nurse in the state of Colorado;
• Current CPR certification;
• Reliable transportation and a valid driver's license;

Compensation and Benefits
The annual salary for this position is $94,200, with opportunities for advancement and professional growth.
