Job Summary:

Desire Home Care, Inc. is seeking a compassionate and experienced Licensed Vocational Field Nurse (LVN) to provide healthcare services to patients with intellectual and/or developmental disabilities in the comfort of their homes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, in accordance with established care plans and physician's orders.
  • Monitor and assess patients' health status and report any changes or concerns to the supervising Registered Nurse.
  • Administer medications and treatments as prescribed by the physician.
  • Provide wound care, catheter care, G-tube care, and other nursing procedures as needed.
  • Educate patients and their families on their conditions, medications, and self-care techniques.
  • Collaborate with other healthcare professionals to coordinate and manage patient care.
  • Maintain accurate and up-to-date patient records, including progress notes, medication administration records, and nursing assessments.
  • Adhere to all regulatory and ethical standards, including HIPAA guidelines and infection control protocols.
Requirements:
  • Active and unrestricted LVN license in the state of California.
  • Minimum of 1 year of experience as an LVN, preferably in a home health setting.
  • Have experience working with patients diagnosed with autism, cerebral palsy, down syndrome, and/or epilepsy.
  • Have experience working in pediatrics.
  • Strong knowledge of nursing principles, practices, and procedures.
  • Excellent clinical assessment and critical thinking skills.
  • Ability to work independently and manage time effectively.
  • Strong communication, interpersonal, and organizational skills.
  • Must have a valid driver's license and reliable transportation.
  • Must have current BLS certificate.
  • Physical and TB clearance valid within the year.
  • Ability to complete documentation via agency EMR system.

Shifts:

Minimum of 6 hour shifts, no more than 8 hours, no holiday shifts. Morning, Afternoon, and Evening shifts available.
