Job Description

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality healthcare services to our patients. As a member of our team, you will have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits.

Key Responsibilities:

  • Conduct comprehensive health assessments, medical history, diagnosis, and treatment planning
  • Collaborate with physicians, case managers, and other healthcare professionals to provide coordinated care
  • Develop and implement individualized care plans to address the unique needs of each patient
  • Provide education and support to patients and their families to promote healthy behaviors and disease management

Requirements:

  • Master's degree in Nursing or Physician Assistant studies
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice
  • Board certification in a relevant specialty
  • 1+ year of experience as a Nurse Practitioner or Physician Assistant
  • Strong communication and interpersonal skills
