The State of New York is seeking a skilled Registered Nurse 2 to join its team as a Community Nurse Specialist. This role offers an exciting opportunity to make a meaningful difference in the lives of individuals and families in need.
Job Summary
This position is responsible for providing direct care services to individuals with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting. The successful candidate will work closely with an interdisciplinary treatment team to develop and implement individualized care plans that promote optimal health outcomes.
Responsibilities
- Directly provide, oversee, and coordinate nursing and health-related care services to a caseload of individuals in a community setting.
- Perform clinical case management activities, including assessment, planning, implementation, and evaluation of care plans.
- Collaborate with other disciplines and healthcare providers to ensure comprehensive and coordinated care.
- Provide nursing-specific clinical, consultative, and instructional assistance to individuals and their families, other disciplines, and healthcare providers.
Requirements
To be eligible for this position, candidates must possess a license and current registration to practice nursing in New York State, a bachelor's degree in nursing, and two years of post-licensure professional clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
Salary Information
The estimated salary range for this position is $120,120 to $129,781 per year, based on location and level of experience.
