Key Responsibilities:- Conduct thorough assessments of members' medical needs and develop proactive plans to address issues presented
- Collaborate with members, healthcare providers, and other stakeholders to ensure seamless care coordination
- Utilize clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators
- Apply knowledge of clinical tools and information/data review to facilitate integrative functions and smooth transitions to Aetna programs and plans
- Active and unrestricted RN licensure in the state of Ohio
- 2+ years of clinical experience as an RN
- Ability to travel up to 50% -75% in Franklin, Delaware, Madison, Pickaway or Union counties
- 1+ year of experience with the Microsoft Office suite
