Position OverviewWe are seeking a dedicated Assertive Community Treatment (ACT) Case Manager to join our team. As a Case Manager, you will provide support to individuals in a community setting, focusing on creating interventions that promote recovery, symptom reduction, and achievement of the highest level of functioning.
Key Responsibilities- Develop and implement individualized treatment plans
- Provide case management services to clients in a community setting
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Monitor client progress and adjust treatment plans as needed
