Key Responsibilities
  • Provide comprehensive care to patients in their homes, including admission, case management, and follow-up visits.
  • Collaborate with our interdisciplinary team to develop and implement personalized care plans that address the unique needs of each patient.
  • Confer with physicians to ensure seamless care coordination and make informed decisions about patient care.
  • Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and home health aides, to ensure a holistic approach to patient care.
  • Document patient progress and communicate effectively with patients, families, and healthcare teams to ensure continuity of care.
  • Supervise and educate home health aides and personal care workers to ensure they have the necessary skills and knowledge to provide high-quality care.
  • Discharge patients when they no longer require home health services, ensuring a smooth transition to other care settings.
Requirements

To succeed in this role, you'll need:

  • A diploma, associate, or bachelor's degree in nursing.
  • At least one year of nursing experience, preferably in a med-surg, ICU, ER, or acute care setting.
  • Current and unrestricted RN licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
