Key Responsibilities:

  • Responsible for maintaining accurate and up-to-date patient records and reporting on patient outcomes.
Requirements:
  • Current RN licensure and experience in home health or post-acute care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic medical records and other relevant software systems.
