Job Summary

Bradford Hills Nursing & Rehabilitation Center is seeking a skilled Certified Occupational Therapy Assistant (COTA) to join our team on a part-time basis. As a COTA, you will work closely with our Occupational Therapist to provide high-quality care to our residents.

Responsibilities
  • Assist the Occupational Therapist in assessing residents' functional abilities and gathering objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change assessments.
  • Implement intervention plans, including ADL training, based on the guidance of the Occupational Therapist.
  • Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
  • Maintain accurate and timely documentation of resident progress and any interventions provided.
  • Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
  • Conduct remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
  • Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, to provide comprehensive care.
Requirements
  • Current certification as a Certified Occupational Therapy Assistant (COTA)
  • Ability to work independently and as part of a team
  • Excellent communication and documentation skills
  • Knowledge of Point of Service (PoS) care delivery and documentation
  • Ability to maintain confidentiality and handle sensitive information
