As a key member of our multidisciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients in their homes, promoting their independence and well-being.
Responsibilities- Conduct home visits to assess clients' physical therapy needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Implement manual therapeutic exercises, gait training, and other interventions to improve clients' functional abilities.
- Collaborate with nursing staff to develop and revise care plans, ensuring comprehensive and coordinated care.
- Educate clients and their families on rehabilitative care and activities to promote healthy living.
- Document observations, interventions, and evaluations using electronic medical records.
- A current Maryland Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records preferred.
