Mass General Brigham is seeking a skilled Registered Nurse (RN) to join our team as a Diabetes Educator. This part-time role will provide diabetes education and care to patients and staff in various settings, including the inpatient Diabetes Care Team at Wentworth-Douglass Hospital.

Key Responsibilities:
  • Provide diabetes education and care to patients and staff in various settings
  • Collaborate with engaged providers and hospitalist team to deliver quality care for optimal outcomes
  • Work with committed clinical staff to assist in discharge planning and continuum of care
  • Participate in Care Team Rounds and consult with nursing staff as requested for inpatient referrals
  • Plan, implement, and evaluate outpatient education programs
  • Participate in health screenings and/or local support group meetings
  • Represent WDH at community events as requested
Requirements:
  • Over three years of experience in an acute care hospital; direct patient diabetes education experience
  • Diabetes Education Certification within 3 years from date in job
  • Registered Nurse License
