Job Summary:We are looking for a dedicated and compassionate Physical Therapist to work with our patients in the South Bay area. As a Physical Therapist with Mission Healthcare, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities:- Provide physical therapy services to patients in their homes, according to a written physician's plan of care.
- Evaluate and assess patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Direct patients and families/caregivers in the use and care of therapeutic appliances.
- Report to physicians on patients' reactions to treatment or changes in condition.
- Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
- Supervise homemaker/home health aides when such personnel participate in patient's rehabilitation regime.
- Identify patient and family/caregiver needs for other home health services and refer as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals.
- Possess a degree from a baccalaureate or master's program in physical therapy approved by an accredited accrediting agency.
- Be licensed to practice as a physical therapist within the state.
- Have at least 1 year of experience as a physical therapist, with community/home health experience preferred.
- Demonstrate good verbal and written communication and organization skills.
- Possess and maintain current BLS Hands On CPR certification.
- A competitive hourly rate of $40-$65.
