Job Summary

Yale New Haven Health is seeking a skilled Registered Nurse to join our team. As a Lead Registered Nurse, you will be responsible for assessing, planning, implementing, and evaluating individual patient care in our clinic. You will work under the supervision of the Nurse Manager and provide supervision to non-RN personnel on the assigned unit.

Key Responsibilities

  • Assess the patient's physical and psychosocial condition on an ongoing basis throughout the continuum of care.
  • Develop, communicate, and document a plan of care reflecting individualized patient problems and interventions to achieve desired outcomes.
  • Implement and coordinate the plan of care in collaboration with the healthcare team.
  • Evaluate the effectiveness of the plan of care, patient's response to care, and readiness for discharge.
  • Plan for education after assessing the individual learning needs and readiness of the patient and family.
  • Function as a preceptor to new staff.
  • Function as a charge nurse.
  • Demonstrate the ability to provide a safe environment for self and patients.
  • Meet performance expectations for customer service, teamwork, resource utilization, and staff and self-development.

Qualifications

Graduate of an accredited school of nursing; BSN preferred. Proficient in the use of computer software to include electronic patient records. Must have the ability to use the internet for literature searches regarding care for patients.

1-2 years' experience preferred. Valid State of Connecticut Registered Nurse License. BLS required.
