  • Discharge Planning: Coordinate with patients, families, and healthcare providers to ensure smooth transitions from hospital to home or other care settings.
  • Utilization Management: Review and authorize medical services to ensure they are medically necessary and meet Kaiser Permanente's quality and cost standards.
  • Community Resources: Connect patients and families with community resources and support services to promote health and well-being.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Education: Master's degree in Social Work from an accredited institution.
  • Experience: Minimum two years of case management experience in a healthcare setting, preferably with a focus on geriatric or complex patient populations.
  • Licensure: Current and valid LCSW licensure highly preferred.
  • Language: Bilingual skills in Spanish highly preferred.
Working Conditions:

This is a part-time position, working 24 hours per week. The schedule will include a mix of weekdays and Saturdays. Travel is required, with approximately 50% of the time spent on the road.
