POSITION SUMMARY

The Certified Home Health Aide operates under the guidance of a registered nurse, executing various tasks as designated in alignment with the patient's care plan to fulfill personal needs and enhance comfort. The hospice aide is tasked with documenting and reporting observations and care provided.

The Certified Home Health Aide will be allocated responsibilities that ensure the quality, continuity, and safety of patient care.

KEY RESPONSIBILITIES

Responsibilities of the Certified Home Health Aide encompass, but are not limited to, the following:

1. Addressing personal care requirements following patient assignments and responding to requests promptly. If a task cannot be performed, it must be reported to the Case Manager immediately.

2. Engaging with patients and families using effective communication techniques, adhering to ethical and respectful care principles, including maintaining confidentiality.

3. Ensuring patient safety and utilizing equipment correctly (e.g., foot stools, side rails, oxygen equipment).

4. Delivering personal care services such as bathing, back massages, oral hygiene, shampooing, and changing bed linens as scheduled.

5. Assisting patients with dressing and undressing as needed.

6. Aiding in feeding patients as assigned, while adhering to regulations regarding food preparation.

7. Monitoring and caring for the patient's skin to prevent tissue breakdown over bony areas.

8. Reporting any pain complaints or changes in the patient's condition to the RN Case Manager, including awareness of interpersonal issues involving caregivers or others living with the patient.

9. Supporting ambulation and exercise as directed by the hospice nurse or therapist.

10. Facilitating normal range of motion and transfers/positioning as instructed.

11. Assisting with self-administered medications as permitted by state regulations, such as reminders.

12. Providing assistance with bedpans and urinals, as well as light housekeeping tasks like laundry and waste disposal.

13. Performing range of motion exercises and other simple procedures as ordered, demonstrating the necessary skills competency.

14. Offering respite care for patients, families, or caregivers when present.

15. Maintaining cleanliness and orderliness in the patient's living space as assigned.

16. Complying with the organization's documentation and care protocols, along with standards of personal and professional conduct.

17. Participating in quality assessment and performance improvement initiatives.

18. Being respectful of the patient and family environment, as well as the personal needs of the patient.

19. As a mandatory reporter, ensuring compliance with reporting requirements to the NJ Department of Health within specified timeframes.

20. Reporting any suspected cases of abandonment, abuse, financial exploitation, or neglect in accordance with NJ State laws and ethics to the appropriate authorities.

The aforementioned statements are intended to provide a representative overview of the primary duties and responsibilities of individuals in this role. Incumbents may be asked to perform additional job-related tasks beyond those outlined in this description.

QUALIFICATIONS

1. Completion of a State-approved Certified Home Health Aide certification program.

2. High school diploma or General Equivalency Degree (GED).

3. Ability to read and follow written instructions and document care provided.

4. Understanding of hospice philosophy and comfort in providing specialized care to terminally ill patients.

5. Previous experience in hospice, acute care, rehabilitative, or long-term care settings; one year of home health/hospice experience preferred.

6. Basic computer skills for documenting in electronic medical records (EMR).

7. Minimum age of 18 years.

8. Valid driver's license with an insured automobile that meets organizational requirements and is in good working condition.
